THE RED DOOR CLINIC · MELLIEĦA
Longevity and lifestyle medicine
Nutrition, movement, sleep, stress and gut health, brought together in one evidence-based plan.

The five areas
A serious plan does not treat these as slogans. Each area is only useful if it connects to your actual week. The notes below are the conversation we think is worth having. They are not a promise that every area is assessed in a single visit, and they are not personal medical advice.
Nutrition
Nutrition questions start with what you already eat, not with a branded diet. Weight, energy, blood results you have been given, and conditions such as gout or diabetes all change the advice. We will not invent a meal plan that ignores a doctor’s instruction. If you need a dietitian, we should say so.
Movement
Movement is the part of this clinic that is already concrete. Strength, walking, and a joint that can do the task you care about are longevity tools with better evidence than most supplements. A personalised exercise programme can stand alone. Inside a wider plan it is the piece we can write, progress and review. Pain is not a reason to skip this area. It is a reason to dose it properly.
Sleep
Sleep loss shows up as pain tolerance, mood and training that will not stick. We ask about position, mattress, hours, and whether pain is what wakes you. A physiotherapy clinic can help with the mechanical side, the shoulder you cannot lie on, the back that prefers one position. It is not a sleep clinic. Snoring with daytime sleepiness, witnessed pauses in breathing, or insomnia that is taking over your life needs a medical conversation as well.
Stress
Stress is not a character flaw, and it is not fixed by being told to relax. In a movement clinic the useful question is how stress changes your pain, your sleep and the week you can stick to. Breathing drills and pacing can be part of a plan. They are not therapy for a mental health crisis. If you are in crisis, contact a doctor or local emergency support rather than waiting on a lifestyle appointment.
Gut health
Gut symptoms, bloating, reflux, a change in bowel habit, are common and they are not all the same problem. A physiotherapy clinic should not diagnose them from a questionnaire. Red-flag bowel symptoms, including bleeding, unexplained weight loss, or a persistent change, belong with a doctor. Gout, which patients also ask about, is a medical condition. Movement and joint care can sit alongside the treatment your doctor prescribes. They do not replace it.
How the programme works
If the programme is running, it should have a visible shape: an assessment, a written set of priorities, and a review. Five areas on day one is usually too many. A good first visit picks the one or two changes that would matter most in the next month, and leaves the rest for later. That is pacing, not a lack of ambition.
Bring what you already know. Recent blood tests, a list of medication, and any plan from your doctor save time and stop us contradicting care you are already in. Until that is confirmed, treat the published physiotherapy fees as the fees for physiotherapy, not as the price of this programme.
Reviews should look at what you actually did, not at a perfect week. If sleep collapsed or pain flared, the plan changes. Supplements, wearables and restrictive diets are not assumed. If they are discussed, you should hear why, and you should hear when they are a poor use of money. We will not sell you a product from the clinic shop as a condition of being seen.
Who it suits
It suits someone who wants a joined-up conversation about habits and is willing to change one thing at a time. It can sit beside physiotherapy if pain or injury is part of the picture. It is a poor fit if you want a detox, a guaranteed biological age, or a plan that ignores your doctor.
It is also a poor fit, for now, if you need the service to be bookable this week and the clinic has not confirmed it is offered. Ask. The honest answer may be that physiotherapy, exercise and ordinary lifestyle advice inside an assessment are available, and that a named longevity programme is not.
Visitors can ask, with the same caveat. A single holiday appointment can start a conversation. It cannot remodel nutrition, sleep and stress in a week. If you live in Gozo or elsewhere in Malta, say how often you can realistically return. A programme you cannot review is a document, not care.

A LITTLE CLARITY
Frequently asked questions
Is this the same as physiotherapy?
No. Physiotherapy assesses and treats pain, injury and movement. Lifestyle medicine, where it is offered, looks more widely at habits. The two can meet, movement is in both, but booking one does not automatically include the other. Ask which appointment you are being offered.
Will you replace my medication?
No. Do not stop prescribed medication because of a lifestyle conversation. Gout, blood pressure, diabetes and similar conditions stay under your doctor. We can talk about movement and joint care alongside that treatment.
How much does it cost?
Until the clinic confirms a price, do not budget from the physiotherapy list alone.
